Competitive pay rate of £17.59 per hour
Full-time position, working 37 hours per week
The duties of this role include:
Manage a caseload of care leavers, providing tailored support based on their individual needs
Collaborate with social workers to co-work Eligible Children in care
Help develop Pathway Plans and conduct regular reviews
Ensure that care leavers receive the necessary support and resources for a successful transition to independence
Deliver group sessions, activities, and personal development programmes for young people
Liaise with internal and external stakeholders to promote joint work and access additional support services
Monitor and support care leaver’s educational achievements, including those in further and higher education
Maintain accurate and up-to-date records and generate reports as required
Relevant experience in working with young people, particularly those leaving care
Sound knowledge of legislation and policies related to young people leaving care
Strong communication and interpersonal skills to engage with young people and build rapport
