To assess and care manage the complex needs of adults with a disability and their carers to ensure their health, well-being and social inclusion.
To liaise with Re-enablement, where appropriate, and other services and professionals to ensure seamless care pathways which safeguard and promote independence.
To promote partnership working which supports the person centred outcomes to be delivered.
To undertake safeguarding investigations as required.
Local Authorities meet their statutory obligations under the Social Services and Well-being Act 2014 and Mental Health Law.
