Social Worker – Older People & Hospital Social Worker
Job Description
Conwy County Borough Council are currently seeking a highly motivated and experienced Social Worker to join their Older People & Hospital Social Work Services team.
The Social Worker will work with the service user, carers and families to provide support and to develop resilience and independence, ensuring that the service user, families and carers are at the centre of decision making about their lives and the outcomes that they want to achieve.
Key Requirements
Social Work Qualified, minimum 3 years post qualified
Registered with Social Care Wales. If not already registered, they will need to register to work in Conwy County Borough Council.

Responsibilities
Work with the the service user, carers and families to provide support and to develop resilience and independence.
Respond to safeguarding concerns in accordance with the Wales interim policy and procedure for POVA.
Ensure that the service user, families and carers are at the centre of decision making about their lives and the outcomes that they want to achieve
Provide support to the Older People & Hospital Social Work Services team
Location & Duration
The Social Worker will work from the office Eirias Park, Colwyn Bay, Conwy. The assignment is for 37HPW, Monday to Friday. There is an expectation that candidates live locally or within a commutable distance as the position is located in Conwy County. The assignment is expected to last approximately 12 weeks with potential to be extended.
