Reablement Enabler
Service care Solution are currently recruiting for a Reablement Enabler in Islington
The Reablement Enabler will deliver personal care and support to people in accordance with their personalised support/Reablement plans. This may include personal care tasks, domestic tasks or exercise programs, developed by therapists to support people to achieve agreed outcomes and add to their rehabilitation or recovery at home.
This is a part time position working 25 hours a week.
Pay rate – £14.14 per hour / £17.38 ltd (umbrella)
Main Responsibilities
As a Reablement Enabler, you will be responsible for:
To ensure that all there is a strengths-based approach demonstrated in the delivery of care and support as detailed in the persons Reablement plan
To carry out Reablement plans and provision of care and support
To report any safeguarding concerns immediately, whether confirmed or suspected, to the Duty Manager in line with the Councils Safeguarding Policies & Procedures
To ensure that the Care Act 2014 is implemented in all aspects of their role
To work with people using our services, carers, families and other professionals to minimise and manage risk whilst enabling the service user to maintain their chosen lifestyle as far as possible
Requirements:
Willingness and ability to travel is essential for this post and flexible working over a 7-day working week will be required to fit in with the needs of the service. This could include regular morning and evening shifts and overnight support
Enhanced DBS
Level 2 NVQ or QCF Level 3
