We are looking for an experienced and compassionate Hospital Social Worker to join our dedicated team in East London.
This is a fantastic opportunity to play a pivotal role in patient care, working within a dynamic NHS environment.
Main Duties:
Deliver high-quality social work services to patients and their families, addressing a wide range of social, emotional, and practical needs.
Work closely with health care professionals, including doctors, nurses, and allied health teams, to create comprehensive care plans that promote recovery and well-being.
Advocate for patients and their families, ensuring their needs and wishes are central to care planning and decision-making.
Carry out thorough assessments, provide crisis intervention, and coordinate discharge planning.
Liaise with community partners, local authorities, and external agencies to ensure patients have access to appropriate support services.
Maintain accurate and up-to-date case records in line with legal, professional, and organisational standards.
Person Specification:
A degree in Social Work (or equivalent) and registration with Social Work England.
Minimum of 2 years post-qualification experience in a hospital setting.
In-depth knowledge of relevant UK legislation, including safeguarding, the Care Act, and mental health law.
Excellent communication and interpersonal skills, with a compassionate and empathetic approach.
Strong organisational and problem-solving skills, with the ability to work both autonomously and as part of a multi-disciplinary team.
Resilience and adaptability to manage a varied and demanding workload in a fast-paced environment.
