Belmont Recruitment are currently looking for an experienced Community Care Officer to work with a Nottingham-based Local Authority on a temporary basis. This is a full-time role working 37 hours per week (Mon – Fri). The role will offer hybrid working following initial training.
The Adult Duty Team works with adults 18 years of age and over with a range of needs. The team supports adults and their carers by completing holistic, person centred assessments and have a strength based approach to interventions. The team work closely and effectively with partner agencies and aim to support adults to maintain their safety and independence in the community for as long as possible, with a focus on early intervention and prevention.
Main Duties:
* To complete Care Act assessments (both face to face or telephone assessments) depending on the level of risk and the needs of the citizen being assessed, with consideration of mental capacity throughout.
* To ensure the delivery of appropriate services to safeguard vulnerable adults and their Families, according to their assessed needs.
* To undertake all levels of Assessments in relation to the needs of adults and their carers against the departments agreed eligibility criteria and in accordance with National Performance Indicators within set timescales.
* To create and implement culturally appropriate care plans and services that demonstrate Flexibility and innovative use of resources and undertaking a care coordination role.
* To undertake case management responsibilities that include proactively managing a caseload 9including elements of risk management), the monitoring, Co-ordinating and Review of care plans and ongoing service provision.
* To work independently, under the general direction of the Team Leader, to meet Deadlines and manage frequently changing circumstances and conflicting priorities.
* To assist other members of the team in carrying out their work e.g. by providing advice, Support and mentoring (as appropriate). The post holder may also be required to convey services users, perform other escorts duties, assist on home visits, collect information etc.
* To maintain and produce professional case records and reports which are structured, clear, accurate, timely and evidence based In accordance with the departments case Recording standards and practice
Essential Criteria:
* Experience of working with adults in a social care setting.
* Experience of working with adults experiencing a range of difficulties relating to learning disability, mental health, substance misuse and physical health problems.
* An awareness of safeguarding and how to identify people at risk, flexibility, resilience and ability to prioritise and adapt to changing situations.
* The ability to undertake adult-focused assessments of need within a person centred approach.
* To have experience of creating and implementing, individually tailored care plans that demonstrate flexibility and innovative use of resources.
* To have experience of costing and commissioning care packages.
