Adults Social Worker
Blackburn with Darwen Council are seeking a Qualified Social Worker for the community social work team. As an Adults Social Worker, you will be predominantly based at Barbara Castle Way health centre in Blackburn. You will be responsible for providing support and assistance to adults with various needs, including but not limited to mental health issues, physical disabilities, and substance abuse.
Key Responsibilities
Conducting assessments of clients’ needs and developing care plans
Providing support and guidance to clients and their families
Liaising with other healthcare professionals and agencies
Monitoring and reviewing clients’ progress and adjusting care plans as necessary
Maintaining accurate and up-to-date records
Requirements
A degree in Social Work or equivalent qualification
At least 3 years of experience in a similar role
Social Work Registration
Enhanced DBS
