Adults Social Worker
Lancashire County Council are seeking a highly motivated Social Worker to join the Preston North and East Older People & Physical Disabilities Team. The successful candidate will be responsible for providing a strength-based, outcome-focused service to individuals in need of care and support. The role is diverse and varied, operating within a busy community team, and requires recent experience of completing Care Act assessments, Mental Capacity assessments, attending Continuing Healthcare MDT’s and Best Interest Meetings, and working alongside other agencies, professionals, and informal carers.
Key Responsibilities
Conducting assessments to identify the right level of care at the right time
Working collaboratively with other agencies, professionals, and informal carers to provide a holistic service
Providing support and guidance to individuals and their families
Using professional judgement and initiative to ensure individuals are provided with the tools to achieve their optimum level of independence
Ensuring compliance with relevant legislation, policies, and procedures
Requirements
Registered with Social Work England
Enhanced DBS
Recent experience of completing Care Act assessments, Mental Capacity assessments, attending Continuing Healthcare MDT’s and Best Interest Meetings
