Provide essential support to older people in the community with mental health issues, physical and learning disabilities.
Conduct Care Act assessments, develop and review care plans.
Manage a complex caseload while ensuring individuals receive the support they need.To be considered for the role, ideally you will have:
Minimum 2 years’ experience in a similar social work setting.
Diploma/Degree in Social Work.
Registered with Social Work England.
