Mental Health Social Worker – Homeless Team – Bracknell Forest
Provide a rapid, practical response to reports of rough sleeping, reaching out to people with multiple and complex needs, providing crisis intervention, support and advice, support people whom may be in a state of crisis and to make appropriate referrals to agencies and be a link between our service and other support services, such as CMHT, ASC & DAAT Under the guidance of the Rough Sleeper and Single Homeless Team Leader, the post-holder will be responsible for supporting rough sleepers and their homelessness. The post holder will be required to conduct early morning outreach and there may be some unsociable hours and to support people whom may be in a state of crisis and to make appropriate referrals to agencies. The post holder will gather information and identify options and solutions to support vulnerable members of rough sleepers and to provide resources and stress management for customers who are struggling with their mental health.
