At Eden Brown Synergy we work closely and are recruiting for Norfolk County Council for a Adult Social Worker to join their hospital discharge team predominantly to the Norfolk and Norwich University Hospital.
As an Adult Social Worker in the Hospital Discharge Team, you will play a vital role in assessing the social care needs of patients being discharged from hospital. Your responsibilities will include coordinating discharge plans, facilitating the smooth transfer of care, and ensuring that patients have the necessary support in place to continue their recovery outside of the hospital setting.
Full-time (37 hours per week) and part-time (18.5 hours per week).
Rate £32 per hour umbrella.
