    I am currently representing Southwark Council, who are offering an initial temporary contract for a period of 3-6 months with a view to be extended or offered a permanent position for the right candidate at a rate which is negotiable dependent upon experience

    We are looking for a Supported Pathways Officer this role will be: SE1 2QH the shift pattern for one of the post advertised is – 08:00-16:30pm (early shift) and 13:00- 21:30pm (late shift) across 5 hostels with shift pattern 08:00 -16:00pm (early shift) 12:00-20:00pm (late shift) the work pattern is Monday -Friday and shifts are on a rota basis

    The right candidate will:

    * Assist residents in securing their next stage housing, providing resettlement support so that they successfully move into and sustain their new home. To provide appropriate floating support for 6 months and to ensure they are linked into necessary support services thereafter.

    * To provide assertive client centred key-working, risk assessment and support planning to a caseload of approximately 10 clients aimed at supporting them to maintain their current accommodation and link into services and meaningful activities which help them to improve their physical and mental wellbeing well-being and life skills

    * Coordinate any necessary support services to assess residents’ ability to live independently and identify which services could assist them to sustain their wellbeing and tenancy and provide a time limited floating support service to ensure residents are linked into appropriate services after moving out

    * Continually monitor and safeguard residents’ physical and mental health and liaise closely with appropriate services e.g. ASC, CMHT, Health Services, Criminal Justice etc and working collaboratively with other services by referring and reporting to ASC, SOVA, MARAC panels and other services involved in your resident’s care.

    We require the following:

    * Experience of working with homeless people with complex support needs

    * Experience of recovery focused and harm minimisation service provision for clients with multiple and complex needs

    * Knowledge of the impact of Adverse Child Experiences and the principles of Trauma Informed Care when dealing with homeless people who have complex needs
