    Key Responsibilities:

    Care Coordination: Oversee complex cases under the CPA, ensuring comprehensive and co-produced care plans.
    Holistic Assessment: Conduct thorough assessments of mental health and social care needs, including Care Act assessments.
    Risk Management: Perform detailed risk assessments and manage safeguarding measures.
    Support Networks: Collaborate with carers, support networks, and other organisations for integrated care.
    Liaison Work: Coordinate with inpatient units and other statutory and voluntary organisations.
    Documentation: Maintain accurate records and produce detailed reports.
    What You Bring:
    Experience: Proven track record of working with adults with psychosis and complex needs.
    Knowledge: In-depth understanding of relevant legislation, policies, and procedures, including the Mental Health Act, Care Act, and Mental Capacity Act.
    Skills: Strong organisational and time management abilities, excellent interpersonal skills, and effective communication.
    Team Player: Ability to work collaboratively with a range of professionals and engage with service users from diverse backgrounds.

    Essential Requirements:

    SWE registration
    Extensive experience in mental health social work
    Proficiency in conducting Care Act assessments and reviews
    Ability to manage time effectively and prioritise workload
