Social Work Officer
Haringey Council are looking for a dedicated and passionate Social Work Officer to join their team. As a Social Work Officer, you will be responsible for supporting vulnerable adults across the borough through the implementation of the company’s strengths-based model of practice. You will be working closely with partners to maximise the use and benefit of community resources and help residents and their carers to maximise their independence, build on their strengths and improve their quality of life.
Responsibilities
Undertake strengths-based assessments, reviews and support planning that enable residents to maximise their independence, improve quality of life and build their resilience.
Provide practical support to residents and carers, that helps support them such as accessing living aids: accessing the welfare benefit system in order to provide advice and information to service users.
Assist in arranging short term care and in providing clients and carers with respite to reduce carer breakdown.
Work with adult clients, relatives, carers and communities to help them make informed decisions, enabling them to clarify and express their needs and contribute to support planning.
Liaise with colleagues across the Haringey health and social care partnership in order to gather information relevant to assessment and support planning activities.
Maintain and update reliable, timely and accurate case notes, systems and other records, write reports as required; support social worker to collect information and present to legal.
Undertake reviews of care packages.
Requirements
Level 5 Diploma in Health and Social Care or equivalent.
Experience in working with vulnerable adults and knowledge of the strengths-based model of practice.
Experience in undertaking assessments, reviews and support planning.
Ability to work effectively with partners and maximise the use and benefit of community resources.
