As a Hospital Social Worker, your primary responsibility will be to support patients and their families through challenging times, addressing their needs in collaboration with a multidisciplinary healthcare team.
Key Responsibilities:
Assessment and Intervention: Conduct thorough assessments of patients’ social and emotional needs. Develop and implement intervention plans to address identified concerns.
Hospital Discharge
Collaboration: Work closely with healthcare professionals, including physicians, nurses, and other allied health staff, to coordinate and enhance patient care. Participate in multidisciplinary meetings to provide valuable input into patient care plans.
Crisis Intervention: Provide timely and effective crisis intervention services to individuals and families facing acute medical situations. Support patients in coping with illness, trauma, and end-of-life issues.
Advocacy: Advocate for patients’ rights and access to resources. Collaborate with external agencies and community services to ensure a seamless transition for patients upon discharge.
Education and Support: Offer emotional support and education to patients and their families, empowering them to make informed decisions about their healthcare.
